Most likely causes
Failed encoder — Internal failure gives the controller no or wrong speed feedback.
Loose or corroded connector — A bad connection causes intermittent cut-outs identical to a dead encoder.
Damaged cable — Chafed or pinched encoder cable shorts or opens the signal.
